We stay involved through the first weeks of care, answering questions and following up to help keep promising matches from stalling on practical details.

How the fee works

$30per completed week, tax included, for up to four weeks.

Applies to commercially insured and cash-pay clients whose match you accept on or after October 15, 2026.

  • Partial weeks aren’t billed.
  • The total Care Start fee is based on the completed weeks – not session volume, your rates, or what you bill.
  • Joining, matching and introductions are free.
  • If we confirm we didn’t provide a service we’re responsible for, we waive or refund the fee for every affected week.Missing our one-business-day reply target alone doesn’t qualify. Tell us about a service problem within 60 days after your Care Start window closes. (Therapist Agreement, Articles 7.1 and 16.3)

When the fee starts

The weekly fee starts the day your client books their first therapy session. If that session is more than two weeks away, it starts two weeks before the session instead. A free consult doesn’t start it.

If no replacement session is scheduled within seven days, the match ends on the day of the missed session, or of an earlier cancellation. There’s no fee for that week or any later week. Weeks that finished before that day are still charged. A replacement scheduled within those seven days, even for a later date, keeps the match going.

For example, the fee starts October 30. The client misses the first session on November 13, and no replacement is scheduled by the end of November 20. The match ends November 13:

It arrives on the 8th, and we charge on the 15th. You can dispute a charge for 30 days after the 15th.

    These HIPAA business associate terms cover how we handle client information for your practice. Someone authorized signs, usually you in a solo practice. If your practice has already signed, confirm you’re part of it.

  3. Your choice

    Choose whether to accept Care Start fees

    Needed to accept commercial-insurance and cash-pay matches on or after October 15.

    If you accept

    Your practice can accept and pay, or you can do so yourself. No card is needed to sign; whoever pays adds one before you accept your first commercial-insurance or cash-pay match.

    Whoever accepts the fees can withdraw that acceptance at any time. Matches already accepted keep their fee terms.

    If you decline

    You stay in the network. Public-program matches remain available, and you can choose to accept the fees later.

Questions about Care Start

Fees and billing

When is Care Start free?

There’s no fee at all when:

  • The client has QUEST, Medicare, TRICARE, VA, or another public program, even as secondary coverage. Employer plans, including EUTF and FEHB, count as commercial.
  • You accepted the match before October 15, 2026.
  • No first therapy session is booked.
  • The match ends in the fee’s first week and the client doesn’t return within 28 days.

There’s no fee for a week when:

  • The match has ended. That includes the week it ends and every week after.
  • We can’t confirm the client’s coverage type for that week.
What if the first session is rescheduled?
Postponing it before it takes place can move the fee’s start later. Moving the session earlier doesn’t start the fee sooner. Once the first session takes place, the dates are fixed. Rescheduling never adds weeks or raises the four-week limit.
What if a client comes back after care ends?

If the client returns to you within 28 days of the ending – a further session is booked and recorded with us, or the client tells us they want to come back – Care Start resumes for whatever remained of the original four weeks. The days in between are never billed, and nothing runs past the original four weeks.

After 28 days, the fee can start again only if the client comes back to you through Breakthrough. Sessions you arrange privately never restart it.

Why charge for this now?
We’ve absorbed the cost of onboarding questions, follow-up, and rematching while building Breakthrough. Care Start makes that support sustainable.
My practice does its own onboarding. Why is the fee the same?

The fee pays for the support you get either way: we answer onboarding questions from you and your client, follow up when onboarding stalls, keep you updated on the status of our work for each new client, and make the profile and match changes you send us by email. If a client wants a different fit, we offer them other options.

Scheduling, intake coordination and insurance verification can each stay with your own team. Tell us which in your portal or by email, and we’ll leave that work to you. If it stalls, we can step in after checking with you.

Do paying therapists get preference in matching?
No. Among therapists eligible for a client, fees and payment history never affect who is shown, how prominently or in what order. Accepting the fees only decides whether you can take new commercial-insurance and cash-pay matches.
What are clients told?
Clients are told that therapists may pay for Care Start. Clients never pay these fees.
Is a number of matches guaranteed?
No. Breakthrough doesn’t guarantee any number of matches, introductions or clients.
